About this event

What help is on offer?

This event has been organised by Selby Town Primary Care Network. They bring general practices together to help people, as well as providing a wide range of services to patients.

The theme of the event is asking what matters to you, and telling you about the support that is available. Things include:

  • Mobility and independence
  • Exercise and weight management
  • Mental health and emotional well-being
  • Financial support
  • Diet and lifestyle

Healthwatch North Yorkshire will be there to seek your views on what’s important when it comes to your experiences of health and social care services. 

We would like to give you the opportunity to hear about Healthwatch and listen to your views on what has worked well and what could be done differently.


What will happen on the day

Arrival: Patients will receive an ageing well health fair passport upon arrival. The first page includes a template for a 'what matters to you' conversation.

Initial questions: Patients will be asked three questions by a team member:

  1. What matters to you today?
  2. What do you want to know more about?
  3. What help and advice do you feel you need?

Patients will be offered additional support. Patients can also talk with the therapy team, including occupational therapists and physiotherapists.

Exploration: Following the initial conversation, patients will either be directed to specific stalls for support or encouraged to explore the fair to find helpful resources.

Refreshments: Selby Town Primary Care Network will have a café area with refreshments, kindly supported by volunteers.
