Selby STAY Well service
STAY Well is a floating support service for people with primary mental health needs living in the Selby District of North Yorkshire, who need housing related support to maximise and sustain their independence in the community, and to achieve their rehabilitation and recovery goals. STAY Well is a home based service, which means support workers visit people in their own home.
STAY Well is for people who meet the following criteria:
Aged 18 or over and living in the Selby District of North Yorkshire.
Have primary mental health needs
Need of housing related support in order to move towards or continue independent living.
STAY Well can support single people, couples and families, with or without children.
The key aims of the supported service is to:
- enable people to stay healthy, safe and well in the community;
- enable people to remain in their own home, wherever possible;
- Build resilience to maximise people’s independence and minimise continuing dependency on service.
- provide good quality information and advice;
- provide services which are flexible enough to respond to people in crisis;
- Reduce demand on the health and social care system, including admissions into hospital or residential care.
- allow people to have an understanding and control of their housing situation, and therefore, to make more informed decisions
The service is free and accepts referrals from any source, including statutory and voluntary organisations and self-referrals.
